How to Become a Patient

Your insurance company may require that you get a referral from your Primary Care Physician before becoming a patient. Please check your policy. If required, you must get the proper authorization from your insurance company before scheduling your appointment.

Scheduling Your First Appointment

If a referral is required, the referring physician may schedule the appointment for you; or, once you receive the referral, you may schedule the appointment yourself by telephoning our office. If your insurance does not require a referral, please call our office directly to schedule your first appointment.

Preparing For Your First Appointment

When you come for your first appointment, please bring the following:

1. Current picture ID
2. Insurance card(s)
3. A complete list of current medications
4. Details of your medical history
5. Method of payment
